If you have one, write it in this blank. If not, you have to figure out how to get one.
Current address and permanent address
These two addresses would be the same if you intend to live at home while working. However, if you live in a private high school or college at the time you are working, write your school address as your current address and your home address as your permanent address.

List “minimum wage” unless you know your experience and skills justify a higher pay. In other words, write down the amount you feel you deserve.
